Members have a choice of more than 6,500 dentists in California and one of the largest networks of providers of any prepaid dental plan - all carefully credentialed to offer the kind of care we demand and our patients expect. Please check with the individual office to determine if they treat children. Some offices may charge a sterilization fee. You must notify the Plan's Member Services Department of any Dental office change before eligibility can be provided. Other dental offices will be added periodically. If any office is closed to further enrollment, the Plan will ask you to select another participating dental office. Please be certain to check your Plan ID Card to see who your participating provider is. Call the dental office to see if they are accepting the PacifiCare 511 plan. Then call PacifiCare (1-800-228-3384) 8am - 6pm Monday - Friday to see if the dental office is active and in good standing. You MUST go to your assigned dentist to receive the benefits. If your dentist advises you of the need to consult a specialist for oral surgery, endodontics or peridontics, you MUST telephone the Dental Plan Member Services Department to get the name and location of the specialist most convenient to you. Only by this method will your procedures be provided at the reduced prices.
